Medicare built a pilot program called the Innovation in Behavioral Health (IBH) Model. It runs from 2025 through 2032. Starting in 2028, it pays practices for behavioral health care that's coordinated across a whole team — including the social side of a person's life, like housing and food.
Here's the good news, and it's about timing. Coordinating across organizations isn't a switch you flip. It's shared records, real working relationships with partners, and consent forms that cover both the health side and the community side. That takes a few quarters to stand up — which means the practices that start now walk into 2028 already doing the thing.
